Temporary/Permanent Adoption Contract



***NOTE*** The contract should be completely filled out and signed. Be sure that your forms of ID are included and that the contract is signed and dated. Send the contract and your check for the Adoption Reimbursement amount, and any shipping fees or charges with the Contract. The dog will not be sent until the contract, reimbursement, and any charges are received by the rescue.


We(I)_______________________________________ agree to take care of _____________________________ , to care for as our own pet. We understand that a reimbursement amount of $ _______ .____ required, and that this reimbursement amount is a non refundable/refundable (circle one) reimbursement amount. The reimbursement amount will be set to cover only the cost of medical treatment of the dog(s), and the spay/neuter of said animal. Return of the animal for any reason could result in the loss/return (circle one) of this reimbursement amount by prospective adoptive parents/adoptive parents. We understand that any and all shipping charges are non refundable.

We understand that full ownership of these dogs resides with Marsha Taylor Harper d.b.a. National Maltese and Small Dog Rescue Alliance and that at any time the dog(s) may be reclaimed if conditions of "Default" apply (See Default), by the owner.

Trial Period:

The trial period for the prospective adoptive parents will be a 14 day period whereby the family can get to know the dog(s). During this time if any vet visits are required they will be made only with the written consent of the owner of the dog(s), Marsha Taylor Harper. Getting unauthorized vet care is considered default. At the end of this trial period the decision will be made by both prospective adoptive parents and owner to be sure that placement is in the best interest of both the prospective adoptive parents and the dog(s). If at this time, the feeling is that his is a suitable home and all are in agreement, the adoption papers will be signed and the dogs will be placed with new conditional ownership. If the home is not approved by the owner, Marsha Taylor Harper, the dog(s) will be picked up and returned to rescue.

Permanent Adoption:

Permanent adoption of the dog(s) still does not give the adoptive parents permission to give away, sell, or replace the dog(s). In the event that the adoptive family can no longer keep the dog(s), the dog(s) shall be returned to the owner, Marsha Taylor Harper d.b.a. National Maltese and Small Dog Rescue Alliance. Failure to return dog(s) to National Maltese and Small Dog Rescue Alliance will result in legal action by which the adoptive parents will be subject to, but not limited to, pay all legal costs, any medical conditions resulting in this placement, and cost to retrieve the dog(s). If adoptive parents have a requested adoptive home, they may submit this home information to National Maltese and Small Dog Rescue Alliance and this home will be the first considered home for adoption.

Default by prospective adoptive home or adoptive home:

Failure to get medical attention when required, failure to keep the dog(s) groomed, failure to allow National Maltese and Small Dog Rescue Alliance to make a check visit on dog(s), or failure to keep any agreement of this contract will be reason for immediate cancellation of all contracts with prospective adoptive or adoptive parents. National Maltese and Small Dog Rescue Alliance reserves the right to visit with dog(s) at any time during their life to assure that the dog(s) is/are receiving adequate attention and are cared for properly. (an appointment will be made, in advance, for visits that are convenient to both parties) Default will result in legal action by which the adoptive parents will be subject to, but not limited to, pay all legal costs, any medical conditions, and cost to retrieve the dog(s).

Death of dog(s):

In the event of death of a dog(s), prospective adoptive parents/adoptive parents are required to notify National Maltese and Small Dog Rescue Alliance within 24 hours from such death, and to also have an autopsy done if death occurs at any time prior to the dog(s) 12th birthday, unless a qualified veterinarian has determined cause of death beyond any doubt. This report is to be given to National Maltese and Small Dog Rescue Alliance within 14 working days from the death of said animal. Failure to comply could be taken as default and all legal issues of default will apply.

Safekeeping of dog(s):

Any and all City, County, State, and Federal laws are to be met and honored by prospective adoptive parents/adoptive parents. Maltese are house dogs and shall be treated as such, maintained in the home, with adequate heat and air conditioning as to keep them comfortable at all times. Under no conditions shall the Maltese be kept outside the home in a yard, separate building or structure not considered a part of the home. Maltese are dogs that require grooming. Prospective adoptive parents/adoptive parents agree that grooming will be done on a timely basis. Timely basis is no more than every 10 weeks.

Lost dog(s):

If at any time the dog(s) are to become lost, National Maltese and Small Dog Rescue Alliance is to be notified immediately. Circumstance of this loss may be considered as default by such.
